2. You receive our questionnaire, you fill it out and send it back.

3. After receiving the completed questionnaire, we will answer you (usually 

     within 1-3 days) and let you know whether we can put you on the waiting list.

4. We make an appointment to get to know you personally (visit lasts approx.1 hour).
